The Functional Health Assessment
Before you can get to where you want to be going, you must first know where you are. The questionnaire below, called the Functional Health Assessment (FHA), quantifies your current health level. Some questions are about family history or one-time events like a traumatic injury. However, the vast majority, are symptoms related to Core 4 imbalances.
The FHA is over 150 questions, divided into eight categories for informational, and not treatment, purposes. To complete the test simply click “yes” if the question applies to you. The "yes" answers from all sections are then added together, converted into a grade value, and superimposed on the Stress Threshold graph at the end of the assessment.
